Social Services Coordinator for adult day health care center. The Daily Living Centers are an Adult Day Health care program developed to be the perfect solution for caregivers who are searching for alternative care for their loved ones with special needs. Our Participants are socially and physically active in an environment that is enjoyable, comfortable, and stimulating.

The Daily Living Centers have a total of four facilities which serve Oklahoma City and surrounding metro areas. The centers average 145 individuals per day. We are committed to providing the highest quality adult day health care in social interaction and medical monitoring to those who may require a more structured and protective environment. Our adult day health care programs are here to help by offering the following services:

  • Socialization
  • Medical Monitoring
  • Promote self-efficacy
  • Caregiver Support Groups
  • Crafts, Games & Contests
  • Health & Wellness
  • Hydro-massage Whirlpool Bathing
  • Door-to-door Limousine and Van Transportation
  • 4 Locations (Edmond, Bethany, South Oklahoma City and Central Oklahoma City

Essential Functions

  • Assists all four centers with applying for and processing financial assistance for program Participants including DHS, ADvantage, VA, etc. Tracks applications and troubleshoots as needed
  • Provides social services support for caregivers and participants
  • Attends professional meetings and represents the organization professionally and effectively in the community
  • Interfaces and guides clients and families to determine care availability, accessibility, and type
  • Ability to work independently under general instruction on a variety of special projects;
  • Develops, promotes and hosts Caregiver support groups for all four centers
  • Creates and meets monthly performance goals

Skills and Abilities Required

  • Ability to interact in a positive, supportive manner and communicate well with staff, referral sources, and the community regarding services provided by Daily Living Centers and the organizations mission, philosophies, goals, and objectives.
  • Excellent oral/written communication skills including professional letter composition
  • Basic knowledge of computer literacy skills
  • Ability to handle multiple projects and tasks simultaneously
  • Adherence to policies and procedures are a must
  • Ability to perform critical thinking, active listening, judgment and decision making, effective speaking, time management, persuasion, persistence, flexibility, and social perceptiveness;

Education and/or Experience

  • Degree required. Social Work preferred, other psych/social degree considered
  • A minimum of 2 years’ experience in adult case management required
  • Must have valid driver’s license and auto insurance
  • Clearance of background check and drug screen

  • Builds relationships and educates families, understanding family needs
  • Manage the sales process guiding families through their search process with post referral follow up
  • Manage leads and utilize computer systems to manage work flow and maintain accurate sales forecasts
