Certification

1. We were informed that our nursing home was selected to participate in the program, what do we do to schedule our certification training?

Once you receive official verification you are accepted in the program, we will send directly to your lead organizers the log in information. You will be given a conference access number: audio is transmitted through your phone, so include a speaker phone if training in a group.

2. How do we receive a certificate and certification plaque?

After your organization has been through all three 90-minutes webinar training sessions provided by Music & MemorySM, you need to contact Music & MemorySM.

• For an individual certificate of participation, email: certificates@musicandmemory.org of the individuals who would like a certificate.

Include:

o Names of participants who have attended all sessions
o Organization name as it should appear
o Contact person and their email to send electronic certificates.

• For a facility plaque, email: plaques@musicandmemory.org

Include:

o Organization name as it should appear on plaque
o Contact person, email address, shipping address and phone number

3. What can we do until we are certified and receive our starter equipment kits?

In addition:

• Identify your Team Members who will be involved. This is a company-wide, transformation program. See agreement for the minimum five members of the team. A diverse team is essential for your success.
• Identify how and where the equipment will be stored.
• Create any needed Music and Memory policies. See sample policies at LeadingAge Oklahoma.
• Check back often as more sample policies will be added as we continue through the program.
• Identify how Music and Memory will be utilized in care planning.
• Introduce Music and Memory to the residents and families at both Resident and Family councils.

4. What are the dates of the certification?

The certification training dates are:

  • April 11, 12, and 13, 12:00-1:30 p.m. 
  • May 16, 17, and 18, 12:00-1:30 p.m.

Each session is approximately 1.5 hours and at a minimum, your entire team must participate in all three days of the webinars. If you would like to participate in both the August and September sessions just let us know and we can send you the access code for each one.

COST

1. What is the cost to participate?

Participation not through the grant:

If your community/organization does NOT qualify for this grant you may register and pay for the Music & MemorySM certification without financial support from LeadingAge Oklahoma. For information about costs and certification benefits visit Music and Memory.

Benefits of participating with LeadingAge Oklahoma:

Non-grant participants may participate in the LeadingAge Oklahoma grant sponsored monthly webinars and educational seminars at a cost of $250.00 per company.

The $250.00 allows for:

• Two staff to attend three dementia seminars during the grant period (ending June 2018).
• Access to the LeadingAge Oklahoma monthly webinars.
• CEUs are available for an additional fee of $35.00.
• Program updates and provider networking to enhance implementation at your community/organization.

Participation through the grant:

As a grant participant there is no cost to participate. Your cost will be the staff time you utilize to implement the program. There is a deposit of $250.00 and is refundable once all program requirements are met.

EQUIPMENT

1. We were told we would be provided with a starter kit, when and how will we receive this?

The equipment will be mailed directly to you. You will be notified of the date.

MUSIC

1. Is there a license required to use CDs or iTunes music?

Music Sharing Terms & Conditions:

We are often asked if there are any copyright limitations when transferring music onto a large number of iPods for noncommercial use. The answer is NO, but certain terms and conditions apply

Please refer to the following verbiage from Apple’s Terms and Conditions. Section (vi) is the key clearance for the MUSIC & MEMORY℠ program. “Unlimited syncing” is based on the following iTunes Terms of Service Usage Rules.

PLEASE NOTE:

1. The term “device” refers to computers or iOS devices, NOT iPods.
2. Restrictions on burning refers ONLY to burning CDs or DVDs.

Usage Rules

(i) You shall be authorized to use iTunes Products only for personal, noncommercial use.
(ii) You shall be authorized to use iTunes Products on five iTunes-authorized devices at any time, except for Content Rentals (see below).
(iii) You shall be authorized to store iTunes Products from up to five different Accounts a time on compatible devices, provided that each iPhone may sync tone iTunes Products with only a single iTunes-authorized device at a time, and syncing an iPhone with a different iTunes-authorized device will cause tone iTunes Products stored on that iPhone to be erased.
(iv) You shall be authorized to burn an audio playlist up to seven times.
(v) You shall not be entitled to burn video iTunes Products or tone iTunes Products.
(vi) iTunes Plus Products do not contain security technology that limits your usage of such products, and Usage Rules (ii) do not apply to iTunes Plus Products. You may copy, store, and burn iTunes Plus Products as reasonable necessary for personal, noncommercial use.
(vii) You shall be able to manually sync a movie from at least one iTunes-authorized device to devices that have manual sync mode, provided that the movie is associated with an Account on the primary iTunes-authorized device, where the primary iTunes authorized device is the one that first synced with the device or the one that you subsequently designate as primary using iTunes.

PARTICIPATION

1. What are the qualifications to participate?

Any Medicare or Medicaid nursing home in Oklahoma is eligible to participate in the grant and LeadingAge Oklahoma membership is not a requirement.

To enroll, eligible nursing homes must complete all five of the following requirements for participation:

  1.  Attend an orientation webinar convened by LeadingAge Oklahoma to provide general information and answer questions about the project, participation, funding and the research component.
  2.  Submit the required Participation Agreement to LeadingAge Oklahoma and the Research Information Summary, completed in full and signed by an authorized representative of the nursing home.
  3.  Submit the Enrollment Survey to LeadingAge Oklahoma, with all required fields completed.
  4.  Submit most recent Facility Quality Measures Report, with all requested information.
  5.  Submit your $250 refundable deposit.

Exceptions to Eligibility: The following types of nursing homes are not eligible:

1. Hospital-based long term care beds.
2. Nursing homes with a primary focus on short stay residents.

Other levels of care may attend the LeadingAge Oklahoma seminars and learn about the program, implementation strategies and best practices. However, only participating providers will receive all of the benefits offered.

2. My Community/Organization Doesn’t Quality for the Grant – How Can I Still Participate?

While the grant is strictly for Oklahoma Medicare/Medicaid nursing homes, LeadingAge Oklahoma is encouraging all senior care providers to participate in Music and MemorySM and the Oklahoma implementation. By doing so, you and your clients will reap the benefits from the well-studied and documented program. The research has demonstrated that music can reduce the agitation of dementia, lessen anxiety, reduce pain, lower blood pressure, help with sleep, increase cooperation and much more. These are things all providers would be overjoyed to have happen with their residents/clients.

If your community/organization doesn’t qualify for this grant, you may register and pay for the Music & MemorySM certification without financial support from LeadingAge Oklahoma. For information about costs and certification benefits, visit Music and Memory.

SIGN UP

1. Is there a deadline to sign up?

The grant is limited to 100 nursing homes – submission of your application and deposit fee is critical to ensure your participation. Phase II Participants must be finalized before the May Certification webinars. Participation does require some advance time to complete training, adopt care plans, implement resident interventions and collect and compare baseline data. Facilities should sign up as soon as they are committed to participation. Spots are filling up quickly!
